IDR Healthcare is seeking a travel nurse RN Home Health for a travel nursing job in Charlottesville, Virginia.
Job Description & Requirements Pay Rate: $55–$59/hr
\n Mileage Reimbursement: $0.45 per mile
\n Schedule: Monday–Friday (Flexible)
\n On-Call: 1 week per month (easy rotation)
\n Coverage Area: Charlottesville to Staunton, VA
We are seeking an experienced Registered Nurse (RN) to provide high-quality, patient-centered home health services. The ideal candidate will be comfortable managing a flexible schedule, completing required daily productivity points, and collaborating with an interdisciplinary team to ensure exceptional outcomes and continuity of care.
\n\n\n Primary Responsibilities \n\nConduct comprehensive assessments for high-risk members and patients in their home setting.
\n\t\n\tCollaborate with primary care providers and other healthcare professionals to develop and implement individualized, goal-oriented care plans.
\n\t\n\tIdentify and coordinate cost-effective, appropriate levels of care across the continuum.
\n\t\n\tFacilitate non-medical supports (housing, transportation, etc.) to promote adherence to care plans.
\n\t\n\tManage and coordinate care transitions to ensure seamless continuity between providers, facilities, and home settings.
\n\t\n\tPartner with physicians, social workers, discharge planners, and claims professionals to support optimal patient outcomes.
\n\t\n\tMaintain detailed documentation of clinical, functional, and financial outcomes.
\n\t\n\tIdentify opportunities for health promotion, illness prevention, and patient education.
\n\t\n\tAct as a patient advocate while maintaining strict confidentiality and HIPAA compliance.
\n\t\n\tSupport the prevention and early intervention of adverse patient outcomes.
\n\t\nEducation: Associate’s Degree in Nursing (BSN preferred).
\n\t\n\tLicensure: Current, unrestricted multi-state RN license (Compact).
\n\t\n\tExperience: Minimum of 1–2 years of clinical experience; home health experience preferred.
\n\t\n\tCertifications:
\n\n\tCurrent CPR/BLS certification
\n\t\t\n\t\tTB test clearance
\n\t\t\n\t\tWound care/competency test completion
\n\t\t\n\t\tCase Management Certification preferred
\n\t\t\n\tTechnical Skills: Proficiency with OASIS documentation and Homecare Homebase (HCHB) required.
\n\t\n\tStrong analytical and decision-making skills in assessing complex medical situations.
\n\t\n\tExcellent communication, time management, and patient engagement abilities.
