Registered Nurse – Home Health
\nWe are seeking a compassionate and skilled Registered Nurse (RN) to join our Home Health team. As a key member of the care delivery team, you will play a vital role in supporting patients on their health journey—right where they feel most comfortable: at home.
\n Position Summary:
\nThe Registered Nurse – Home Health provides personalized, high-quality nursing care to patients in their homes. This role allows for autonomy in your practice, offers flexible scheduling, and provides the opportunity to build meaningful relationships through 1:1 patient care. You’ll work closely with a supportive interdisciplinary team to coordinate and deliver the right care at the right time.
\n Key Responsibilities: \n

    \n\t
  • Perform initial and ongoing assessments of patients’ physical, psychological, social, and environmental needs.\n\t
  • Develop and implement individualized care plans in coordination with patients, families, and the interdisciplinary team.\n\t
  • Reassess patients regularly and adjust care plans as needed based on changes in condition or circumstances.\n\t
  • Communicate significant findings and changes in patient status to supervisors, physicians, and other care team members.\n\t
  • Maintain accurate clinical documentation in compliance with regulatory and organizational standards.\n\t
  • Uphold patient confidentiality and privacy in accordance with HIPAA and other regulations.\n\t
  • Collaborate with other clinicians to ensure coordinated, efficient, and high-quality care.\n
\n Qualifications: \n\n
    \n\t
  • Current, unrestricted RN license in the state of practice\n\t
  • Minimum of 1 year of nursing experience in a clinical care setting\n\t
  • Valid driver’s license, reliable transportation, and active auto insurance\n\t
  • Willingness and ability to travel within assigned service area (up to 50%)\n\t
  • Associate’s or Bachelor’s Degree in Nursing (preferred)\n\t
  • Prior experience in Home Health or Hospice (preferred)\n\t
  • Familiarity with HomeCare HomeBase or similar EMR systems (preferred)\n

IDR Job ID #478510. Pay package is based on 8 hour shifts and 40 hours per week (subject to confirmation) with tax-free stipend amount to be determined.
