Homeless Prevention Case Manager (Bronx, NY) Client Available Hours Full Schedule: Monday - Friday 9am-5pm Rate: $27.00 and up Summary of Client's Description of This Opportunity As a case manager, you will play a critical role in helping individuals and families at risk of homelessness stay stably housed or relocate to stable housing. Your responsibilities will include screening and assessing client needs and developing strategic interventions to stabilize clients' housing. You will be expected to engage clients, assess for program and/or subsidy eligibility, develop service plans, conduct Housing Court advocacy, document progress, complete virtual/in‑person unit inspections, and follow up with external parties. You will connect clients with resources like rental arrears assistance, public benefits and housing vouchers. You will manage a caseload of approximately 40-45 clients in various stages of a housing crisis. We are looking for a desire to work with an at‑risk population, strong assessment and analytical skill. Our team offers a standard Monday‑to‑Friday schedule and the potential for one work‑from‑home day per week. Client's Required Skills & Experience Bachelor's degree in social work or another related field of study. Experience working with homeless population. The position also requires a demonstrated ability in verbal and written communication, including basic computer skills.
